Summer Camp Information Form

Thank you for choosing Izba-Chitalnya Summer Camp! We’re excited to spend this summer with your child. 

To help us prepare everything properly, please complete this short questionnaire. Based on your answers, we will prepare and send your invoice by email.

1. Aftercare (3.30- 6 PM)

Aftercare is available Monday–Friday.
Cost: $50 per day.

2. Lunch Orders

Lunch is optional and costs $15 per day.

Each Friday we will send the lunch menu for the following week by email.

Do you plan for your child to attend aftercare?
How would you like to handle lunch for your child?
If yes, which days will your child attend aftercare?
Will this schedule apply to all weeks you booked?

3. Swimming Pool (Aftercare Only – Mondays & Wednesdays)​
The complex has a pool (3–6 ft depth) and jacuzzi. Children are supervised by two adults, must wear swim vests, and parents provide a vest, towel, goggles, and pool sandals.

Pool participation is optional and free of charge.

If your schedule varies, we will follow up with you by email to confirm the details for each week.
Do you give permission for your child to participate in pool time during aftercare?
